Toronto Breast Implant Removal – Overview and Techniques

Breast implant exchange may be desired for some patients, or implant prosthesis can be removed completely. Implant rupture, heavy breasts, and capsular contracture (thickened scarring around the implant) are common reasons for removal without replacement. Comparatively, desiring a different size, change of placement (over or under the muscle), and various types of implants, such as saline or silicone, are common reasons to opt for implant exchange surgery.

The procedure takes roughly 1 to 2 hours, depending on the complexity. You may choose to schedule a breast lift (mastopexy) simultaneously, which will alter the OR time, cost, and recovery for explantation.

The incisions used are typically placed in the natural breast crease, regardless of the original incision sites, as this provides the least complicated access and visibility. Once healed, the IMF incisions usually appear well hidden and faint.

En bloc breast implant removal is the most common technique used. It means “in one piece” and describes the removal of breast implant devices along with surrounding soft tissue (scar capsule). In cases where undesired thickening or tightening has occurred, if there are unusual cell changes or if implant malposition is being revised, the capsule will be removed entirely. The en bloc technique became important in decades past when many implants contained liquid silicone and ruptures contaminated surrounding tissue. It’s still the safest way to explant most implant ruptures today.

Who is an ideal candidate for breast implant removal?

Patients may decide that they no longer wish to have breast implants for any number of personal reasons. Sometimes the implant choice made many years ago feels too large or uncomfortable in the present. Sometimes patients develop implant capsule tightness and discomfort on one or both sides. If an implant ruptures, it’s recommended that both the implant and the capsule be removed or replaced.

A note about BIA-ALCL

Many people have questions about breast implant-associated anaplastic large cell lymphoma (BIA-ALCL). This is a rare type of immune system-related cancer; it is not breast cancer. This type is specific to the breast implant capsule and seems to be most often associated with textured breast implants. Fewer than 10 patients per year are diagnosed with this condition, and after confirmation through diagnostic testing, the prescribed treatment is typically to remove the breast implants and the surrounding scar tissue. Women should report changes in size or appearance of breasts to their primary care physician and schedule an assessment.

What is implant removal recovery like?

Each patient’s procedure is customized for them, but there are some standard elements to recovery thatyou can expect.

The procedure usually takes 1 to 2 hours, and you’ll be able to head home with a family member or friend as soon as you feel up to it.

You’ll have steri strips (medical tape) covering your incisions and a gauze dressing placed over that. You can sponge bath or shower the day after surgery, depending on your surgeon’s instructions.

Medication to reduce inflammation and discomfort is prescribed, and you may receive an antibiotic prescription. Most patients describe recovery after a breast implant removal procedure as much easier and faster than their original breast augmentation.

Plan for a week off work and at least a few more weeks after that with activity limitations such as swimming or upper body workouts.

It can be helpful to ask a friend or family member to help you with driving and child or pet care in the first two weeks.

Talk to your surgeon about returning to the gym and other activities that you have planned.
